Information on the USASA Player Accident Insurance Program

When you become part of NMSSA, you have access to accident insurance coverage. This insurance is a reimbursement or supplemental insurance that covers up to $5,000. It is provided by US Adult Soccer Association (USASA) and the insurance provider is A&G.

Genral Information Regarding Insurance:

How do I get coverage?  If you are registered with NMSSA, have signed the USASA waiver, and paid the annual fee you are covered.

 

How do I file a claim? 

1. Go to usadultsoccer.com and click on Insurance at the top of the page.

2. Find the "click here to file a claim" in RED to go straight to the online form. (Paper forms are no longer accepted.)

3. Fill out the form in its entirety. Any missing information will result in an incomplete form. Any incorrect information will result in denied coverage.

Who can file a claim?

1. Any player who will seek medical attention or treatment within 90 days of an injury.

2. An injury must have occurred at a sanctioned game, tournament, practice, or while driving to and from a sanctioned soccer event. 

**An injury during a non-affiliated event will not result in coverage. If a player gets struck with lightening while in their car at a soccer event, that is also covered. Insurance does not cover lightening injuries if a player is on the field.**

 

What happens once I file a claim?

1. The form goes to the Verification Officer within your state. This person can verify that you are current with NMSSA.

2. Then the claim gets sent to USASA for them to process.

3. USASA then sends it to the insurance company called A&G.

4. Players will receive an email once the claim has been verified within NMSSA, once it has been approved by USASA, and will be emailed by A&G with a claim number. 

5. A&G will also send you notice in the mail within 1-2 weeks of filing the claim.

 

What does the $5,000 policy cover?

1. In excess of primary insurance coverage.

2. After primary insurance, players will pay a $400 deductible.

3. After the deductible, the policy kicks in and reimburses the player up to $5,000.
